If your clinic uses the TELUS CHR patient app (UpPatient), you can require new patients to complete one or more mandatory intake Qnaires before they can book appointments using the patient app.

As with other Qnaires, the intake Qnaire can update the patient's demographic information and health history in their TELUS CHR chart.

Steps

1. Create the intake Qnaires. See Creating Questionnaires (Qnaires).

2. In the TELUS CHR main menu, click the Qnaires icon.

3. At the top of the Qnaires area, select Self Qnaires.

4. At the top of the Self Initiable Qnaires Window, click on the + icon, and then select the intake Qnaire you created.

You're prompted to select a user (inbox) to receive the filled-in intake Qnaires.

6. Click Tap here to assign, and then select the user.

📌 Note: If you want to require new patient app users to complete more than one Qnaire, you can add multiple self-initiable Qnaires.

7. Click Save.

8. In the TELUS CHR main menu, click the Settings icon.

9. Select Patient Portal.

10. Under Patient App Configuration, click Set intake qnaire, and then select your intake Qnaire.

11. Repeat the above step for any additional mandatory intake Qnaires.

12. Click Save App Settings.
