The TELUS Collaborative Health Record (CHR) has hundreds of the most commonly used provincial and local forms, such as lab and diagnostic imaging requisition forms. TELUS Health creates and maintains these forms for all of our customers.

To add new forms to your CHR, search for and download forms in the Clinical Content Library. Refer to Adding new forms using the Clinical Content Library.

When a new version of a Clinical Content Library form is available, a red alert on the Settings menu option notifies you of new forms. You can then update your existing form. Refer to Updating Clinical Content Library forms.
