TELUS Collaborative Health Record (CHR) Qnaires can score patient answers and produce totals. If patients complete Qnaires more than once, you can track and visualise their score and see how responses to specific questions change over time.

This scoring feature is mainly used for standardized assessment tools, tracking patient outcomes over time. You can also use it for customised assessment tools specific to your clinic. Examples of frequently used scored Qnaires in the CHR include the PHQ-9, GAD-7 and SCAT-5.

📌 Note: You can use Multi-select, Radio, Select Box, Button, Slider, Numerical and Star questions to score responses in Qnaires.

You can score an entire Qnaire (total score) or separate sections of a Qnaire (sub-scores).

These scores can be automatically added to patient data and therefore to an encounter, form and/or letter. See Adding Qnaire scores to patient data.
