TELUS Collaborative Health Record (CHR) Qnaires can score patient answers and produce totals. If patients complete Qnaires more than once, you can track and visualise their score and see how responses to specific questions change over time.

This scoring feature is mainly used for standardized assessment tools, tracking patient outcomes over time. You can also use it for customized assessment tools specific to your clinic. Examples of frequently used scored Qnaires in the CHR include the PHQ-9, GAD-7 and SCAT-5.

๐Ÿ“Œ Note: You can use Multi-select, Radio, Select Box, Button, Slider, Numerical and Star questions to score responses in Qnaires.

You can score an entire Qnaire (total score) or separate sections of a Qnaire (sub-scores).

These scores can be automatically added to patient data and therefore to an encounter, form and/or letter. See Adding Qnaire scores to patient data.

Steps

  1. Add score modifiers to the answer options (choice options) of the Qnaire questions. See Adding score modifiers to questions in Qnaires.

  2. Click Qnaires from the main menu.

  3. Search for the Qnaire you want to add scoring to and click the trend icon next to it.


    The Custom Scoring window appears.

  4. To configure sub-scores in a Qnaire:
    โ€‹
    a. Click +Score.
    โ€‹
    A S1 (Score 1) box appears.


    b. To create a sub-score formula, click the grey box under S1. Select questions from the list on the right hand side and use your keyboard to add + (add), - (substract), * (multiply) and/or / (divide) between each question, as required. Use brackets when dividing or multiplying.

    c. To rename the score, click the Score 1 field, delete the text and type the name of your custom score. This appears on the answer sheet in the patient's chart.

    d. Click Maximum Score and type the maximum amount of points patients can score in this section.

    e. To add another sub-score section, click +Score again. A S2 (Score 2) box appears.

    e. Follow the same steps as above (step 4b - d).

  5. To configure a total score in a Qnaire:
    โ€‹
    a. Click inside the white box under Total Score.
    โ€‹
    b. Select questions from the list on the right hand side and use your keyboard to add + (add), - (substract), * (multiply) and/or / (divide) between each question, as required. Use brackets when dividing or multiplying.

    โ€‹๐Ÿ“Œ Note: If you want to use sub-score totals, click the arrow icons in each section.
    Pink bubble icons appear in the Total Score section.

    You can combine sub-scores and individual questions in a total score formula:

    c. To rename the score, click Total Score, delete the text and type the name of your custom score. The name appears on the answer sheet in the patient's chart.

    d. Click Maximum Score and type the maximum amount of points patients can score in the Qnaire. This appears in the answer sheet of the patient's chart.

  6. Click Apply.
    An Alert window appears notifying you that your changes have been updated.

  7. Click Okay.
    โ€‹

  8. To delete any configured scoring, click the trash can at the top right corner of the Custom Scoring window.
