When you're away from your workstation, you can use your mobile device to create a new encounter and quickly add notes on a patient visit to the History, Examination, Assessment and plan, and Attachments sections.
📌Note: Encounters in TELUS CHR Mobile are read-only. Once you create an encounter, you can update or change the encounter in CHR desktop.
Steps
1. Select the Patients icon from the main menu, then search for and select a patient. For more information, refer to Searching for patients in TELUS CHR Mobile.
2. From the patient chart, select Encounters.
3. The list of draft and signed encounters for the patient is displayed. To create a new encounter, select the plus icon.
4. The New Encounter window opens and shows only the sections you can record information in.
5. To change the date and time of the visit, select the Visit date, choose the appointment date and time, and select OK.
6. To choose from a list of the patient's appointments, select Related Appointment.
7. To record your encounter notes, use the table below for reference.
Section header | Description |
History | Type to record the clinical history associated with the visit.

Examination | Type to record objective information based on physical exam findings.
Select the gear icon in the top-right corner to open the Examination Settings window to decide what vitals you want to display and in what order. 📌Note: When you open these settings for the first time, they match the user’s settings in the CHR. Any changes you make to these settings in CHR Mobile remain until you modify them.
To record standard measurements, including vital signs, select +Add Vitals.

Assessment and plan | Type to record the clinical assessment and recommended treatment plans given to the patient.
To add a new diagnosis, select Add Diagnosis.

Attachments | To attach a file associated with the patient (such as an image file), select +Add Attachment.

8. To create the draft (unsigned) encounter, in the top-right corner, select Create.
📌Note: You can only view existing encounters in TELUS CHR Mobile.
