Preventive care exclusion criteria

In the CHR, you can exclude patients from specific preventive care measures when they should not receive certain screenings or tests. Exclusions can be applied for various reasons including specific health conditions, ongoing specialist care, or patient choice.

To exclude a patient from a specific test or vaccine, use the Excluded Date and Excluded Reason fields in the patient’s chart Patient Data > Preventive Care section. The exclusion or refusal is applied from the date you specify. For more information on updating these fields, refer to Reviewing and updating a patient's preventive care compliance.

Using the following table, you can view the available exclusion reasons and their exemption period for each test.

Test

Excluded Reason

FIT Test

  • Known cancer (1 year exemption)

  • Inflammatory bowel disease (1 year exemption)

  • Malignant bowel disease (permanent exemption)

  • Blood (either bright red or very dark) in the stool (3 month exemption)

  • Unexplained weight loss (3 month exemption)

  • New and persistent diarrhea, constipation or feeling that the bowel does not empty completely (3 month exemption)

  • New and persistent abdominal discomfort (3 month exemption)

  • Patient refused (1 year exemption)

Colonoscopy

  • Being followed by specialist (1 year exemption)

  • Known cancer (1 year exemption)

  • Patient refused (1 year exemption)

Flex Sigmoidoscopy

  • Signs or symptoms suggesting colorectal cancer (CRC) (1 year exemption)

  • Being followed by specialist (1 year exemption)

  • Known cancer (1 year exemption)

  • Patient refused (1 year exemption)

Mammogram

  • Patient has had a mastectomy (permanent exemption)

  • Patient does not want to undergo mammogram (1 year exemption)

  • Patient is being treated for clinical breast disease (1 year exemption)

  • Patient is part of screening program and waiting for an appointment (3 month exemption)

  • Patient refused (1 year exemption)

Pap Smear

  • Patient has had a total hysterectomy (permanent exemption)

  • Patient is being treated for cervical disease preclude PAP (1 year exemption)

  • Patient is not sexually active (1 year exemption)

  • Patient refuses PAP test (1 year exemption)

CV Risk Assessment

  • Patient refused (1 year exemption)

Bone Densitometry (BMD)

  • Patient refused (1 year exemption)

Prostate Specific Antigen (PSA)

  • Patient followed by specialist (1 year exemption)

  • ‌Patient has prostate cancer (1 year exemption)

  • Patient refused (1 year exemption)
