Reviewing and updating a patient's preventive care compliance

If your Collaborative Health Record (CHR) is set up for preventive care tracking, from the Summary view of a patient's chart under the Patient Data > Preventive Care section, you can track compliance on several preventive care measures. Based on province-specific guidelines, items are flagged with a coloured background when a patient is due (i.e. non-compliant) for a preventive care test or vaccine.

From here you can also manually record preventive care information. This allows you to update test dates, record exclusions, and manage patient-specific compliance requirements. These entries are reflected in the Preventive Care Report dashboard.

⚠️Important: When adding patient files related to preventive care measures to the patient chart, use the standard preventive care file names to ensure they are captured in the Preventive Care Report dashboard. For more information, refer to Categorizing preventive care patient files.

Steps

1. From the summary view of the patient’s chart, in the Patient Data section, expand the Preventive Care section. A list of preventive care tests and vaccines appears.

💡 Tip: If you don’t see the Preventive Care section and all the preventive care tests and vaccines, click the gear icon next to Patient Data and add the Properties. For more information, refer to Customizing the patient summary.

2. To update a preventive care test or vaccine, click the patient data property, for example, FIT Test.

3. Using the following table, update the items:

If the test or vaccine has been ordered for the patient but not yet administered, record the date the test was ordered.

Updated Alert Interval (months)

All except Influenza and Pneumovax

To set a customized compliance timeframe that differs from provincial guidelines, type the number of months.

For example, you may want a patient to undergo a pap smear every 12 months after an abnormal result instead of the provincial standard.

Excluded Date

All except Influenza and Pneumovax

If the patient is excluded from the specified test, select the start date from when you want to exclude the test.

For example, if you choose an Excluded Reason that indicates it is a 1 year exemption, the patient is excluded for 1 year from this date.

From the list, select the reason the patient is excluded. The available reasons are based on the specific preventive care measures.

For a list of standard exclusion reasons refer to Preventative care exclusion criteria.

📌Note: To exclude a patient from an influenza or pneumovax vaccine, use the Refused date and Refused reason fields.

Comments

Type any additional comments relevant to the preventive care measures.

Date Notified

Only Influenza and Pneumovax

If you notified the patient that they're due for this vaccination, but they haven't yet received the vaccine, record the notification date.

Refused Date

Only Influenza and Pneumovax

If the patient refused the vaccination, record the date of refusal.

Refused Reason

Only Influenza and Pneumovax

If a patient refused the vaccination, record the reason for refusal.

Type of Scan

Only Bone Densitometry

Select Hip DX, Lumbar Spine DXA, or Femoral Neck DXA from the list.

4. Click Save.
