Collecting patient data using TELUS CHR Intake on a cell phone

Once you downloaded and logged into your clinic's TELUS Collaborative Health Record Intake, patients can complete assigned Qnaires using a cell phone (or tablet) while at your clinic. New patients can register by answering basic demographic questions before completing their assigned Qnaires.

Before you hand the device to a patient to complete assigned Qnaires, you must select a patient, the provider who will receive the Qnaire responses in their inbox, and select the Qnaire(s) that the patient must complete.

Steps

1. Once you are logged into TELUS CHR Intake on your cell phone, select Collect Patient Data. The Collect Patient Data screen appears.

2. Select the patient:

a) Under Select Patient, select Search. The Patient Search screen opens.
b) To search for an existing patient, start typing the patient name in the search field and select the patient.

πŸ“Œ Note: You can search by Full Name (default), Last Name, First Name, ID. To the left of the search field, select an option from the dropdown list.

c) To collect data from a new patient, at the bottom of the screen, select Create a New Patient.

The Collect Patient Data screen appears again.

3. Under Select Destination, tap Select and, from the Select Destination screen, select the provider to receive the Qnaire responses in their inbox.

4. Select the Qnaire:

a) Under Select Questionnaire, tap Select.

b) From the Select Questionnaire screen, type the name of a Qnaire in the search field and select one. Or, under the search field, swipe across the displayed Qnaires and select one.

πŸ“Œ Notes:

  • To add more than one Qnaire, repeat the above step. Patients will be asked to fill out the Qnaires in the order in which you select them.

  • To remove an already selected Qnaire, search for it and tap to deselect it.

5. Select Done.

πŸ“Œ Notes:

  • If you are using a cell phone, the Chat mode is recommended and is selected by default.

  • To modify the assigned patient, provider, or Qnaire(s), under Collect Patient Data select the one to change.

  • To remove all your selections, select Start Over or select the arrow in the top-left corner to return to the home screen.

6. Select Start Questionnaire.

7. Hand the device to the patient, who can now start to complete their assigned Qnaire(s). Advise the patient that once they answer a question, they must select the purple arrow in the lower-right corner of the screen to advance to the next question.

πŸ“Œ Notes:

  • If your TELUS CHR Intake is set to give your patients an option between English and French, a Language screen appears. The patient must swipe up or down to select EN or FR and tap Confirm. See Customising your TELUS CHR Intake settings.

  • A welcome message with or without your clinic's privacy and consent terms appears on the screen. We recommend you add your own privacy and consent terms or the TELUS Health privacy and consent terms. See Adding privacy and consent terms (disclaimer) to your Qnaires.

  • If you selected Create a New Patient above, the patient is now prompted to answer demographic questions before completing the assigned Qnaire(s) (first name, middle name, last name, primary identification, date of birth, sex).

  • If the patient is unable to complete the Qnaire(s) and you want to save the responses (e.g. the patient has to see the clinic nurse), in the top-left corner of the screen, select X. The responses are saved for 24 hours after which they are automatically removed from the app. For more information, see Saving and retrieving incomplete Qnaire responses in TELUS CHR Intake.

8. When your patient has completed the assigned Qnaires, a Success screen appears. The patient hands the device back to you.

9. Select Go Home. An Enter passcode screen appears.

10. Enter your clinic's TELUS CHR Intake passcode to return to the home page of the app.

πŸ’‘ Tip: If your patient needs help while completing Qnaires, in the top-right corner, select ?. They can view frequently asked questions or select I Need More Information to navigate to a web page that you can customize. See Customizing TELUS CHR Intake.
