When you sign an encounter, an Addendums section appears just above the signature. This is the only place where you can add notes to the encounter without unlocking it.

You cannot add addendums while an encounter is in an unlocked state.

You might add an addendum, for example, if you need to edit a signed prescription. You can cancel the prescription, create a new prescription, and enter an addendum in the encounter explaining why the prescription was cancelled.

If multiple addendums are added to an encounter, they appear in chronological order with the newest addendum at the bottom of the list.


1. At the bottom of the signed encounter, click + Add Addendum.

2. An Add Addendum window opens. Enter addendum text and click Save.

3. A confirmation message appears. Click Okay to confirm.

Your addendum content appears at the bottom of the Addendums section of the encounter.

⚠️ Important: You cannot change or delete an addendum after it has been added, even if you unlock the encounter.
