Creating a fake patient

You should have at least one fake patient in your TELUS Collaborative Health Record (CHR). The fake patient can be used for:

  • Deleting unmatched lab reports (see Managing unmatched lab reports).

  • Sending or printing a form that is not associated with a patient.

  • Testing workflows.

  • Internal training.

Steps

  1. From the main menu, click Patients.

  2. In the top-right corner, click New Patient. The New Patient window opens.

  3. From the General tab, enter the fake patient's demographic information. First name, Last name, Date of birth and Sex are required.

  4. Click +Add New Identification. The Create Identification window opens.

    📌 Note: If using a fake provincial health card number as identification, ensure it's in the same format as the province indicated or follow steps 5-9.

  5. Under Create Identification Type, select Private ID, or any identification that is not linked with a provincial issuer (OHIP, MSP, etc.).

  6. Under Identification Value, enter a fake number, for example 1234.

  7. Click Okay. The window closes.

  8. Under Identifications, beside the fake ID created, click Primary.

  9. Click the trash can icon to delete the empty provincial identification field.

  10. Click Save.

💡 Tips:

  • Choose patient names that cannot be confused with real patients.

  • Keep the names professional and friendly, e.g. "Fakey McFakerson".

  • For easy reference, if the fake patient has a specific purpose, use that in the name, for example "Form Vault".

  • If a provider wants to store forms or other information in the fake patient's chart, use the provider's real first name and use "Fake" or "Forms" as the last name.
