Patient data confidentiality (Quebec)

For legal and security reasons, a message concerning the protection of patients' personal information displays the first time a user logs in to the Collaborative Health Record (CHR). By default, this message is configured so that all users are required to accept it before beginning to use the CHR. When the message displays, the user must select the I accept checkbox and click Continue to log in.

You can configure the message so that users are required to accept it periodically, for example, once per year. The content of the message and the last date a user accepted the terms are retained in the user's security settings. For more information, refer to Creating clinic-wide login messages for users.

Responsibility of the user

It is the responsibility of clinic staff to become familiar with this guide and comply with the provisions set out in sections 64 and 65 of the Act respecting Access to documents held by public bodies and the Protection of personal information (https://www.legisquebec.gouv.qc.ca/fr/document/lc/A-2.1).

This Act applies to documents held by a public body in the exercise of its functions, whether their preservation is ensured by the public body or by a third party. It applies regardless of the form of these documents: written, graphic, audio, visual, computerized, or other.

You must always ensure that the information collected comes from the individual in question.

When the patient arrives, request the following information from them:

  • Health insurance number

  • Last name, first name, and date of birth

  • Home address

⚠️ Important: Never disclose patient information.
