In the same way that providers can sign encounter notes, they can sign Qnaire responses. Signing the Qnaire response locks the contents and acts as a digital signature. This is helpful for consent forms that require both the patient’s and provider's signatures.

📌 Notes:

  • You cannot unsign a Qnaire.

  • If the Qnaire response is attached to an encounter note, when you sign the encounter note the Qnaire is automatically signed.

When the Qnaire is signed, the following happens:

  • Patient answers can no longer be modified.

  • The Note section can no longer be modified.

  • The Qnaire response header shows the date of the signature and who signed.

  • The signature and a timestamp appear at the bottom of the Qnaire response.

  • The signature and a timestamp appear in the generated PDF of the Qnaire response.

  • You can no longer delete the Qnaire response from the Qnaire Responses section of the patient chart; however, you can remove it from an encounter note.

Steps

  1. Open a Qnaire response. Refer to Viewing Qnaire responses.

  2. In the top-right corner, click Sign and a signature window opens.

  3. Type “SIGN” in uppercase in the blank field.

  4. Click Sign and the Qnaire response is signed.
