Preventive care management (Alberta and British Columbia)

The TELUS Collaborative Health Record (CHR) enables you to track and manage patients who are overdue (non-compliant) for a number of preventive care measures.

If you review and update your patients' preventive care screening compliance during each visit, using the preventive care analytics dashboards, you can monitor and manage patients overdue for tests and vaccines in one convenient place.

The following table outlines the tests and vaccinations tracked, and how patients are flagged as non-compliant.

📌 Notes:

The information provided is our standard configuration. Your dashboard name, reports and criteria (age, gender, etc.) may differ. Use the information in your CHR as your primary reference. You can find the specific criteria in the analytics tiles.
Patients flagged as being excluded from a test won't show as non-compliant.

Report	Patients are flagged as non-compliant if they...
Blood Pressure	Are at least 18 years old. Haven't had a blood pressure reading in the past 12 months.
Height	Are at least 18 years old. Have never had their height measured.
Weight	Are at least 18 years old. Haven't had their weight measured in the past 3 years.
Exercise Assessment	Are at least 18 years old. Haven't had an exercise assessment in the past 12 months.
Tobacco Use Assessment	Are at least 18 years old. Haven't had a tobacco use assessment in the past 12 months.
Influenza Vaccination	Are at least 18 years old. Have not received an influenza vaccination in the past 12 months.
Mammography	Are female. Are between 45 and 74 years old. Haven't had a mammogram in the past 2 years.
Colorectal Cancer Screen FIT Test Flex Sigmoidoscopy Colonoscopy 📌 Note: The patient needs to complete one of these tests to be compliant.	Are between 50 and 74 years old. Haven't had any of the following tests: A FIT test in the past 2 years. A flex sigmoidoscopy in the past 5 years. A colonoscopy in the past 10 years.
Pap Test	Are female. Are between 25 and 69 years old. Haven't had a pap smear in the past 3 years.
Plasma Lipid Profile Non-Fasting HDL Cholesterol LDL Cholesterol Triglycerides 📌 Note: The patient needs to complete one of these tests to be compliant.	Are between 40 and 74 years. Haven't had any of these specific blood tests in the past 5 years.
Diabetes Screen HbA1C Fasting Glucose 📌 Note: The patient needs to complete one of these tests to be compliant.	Are at least 40 years old. Haven't had any of these specific blood tests in the past 5 years.
