Accessing the OH eForms service

⚠️ Important: This is currently a beta feature, available to a few CHR beta users in order to test and provide feedback. It will be available to more users once the beta period is complete.

Ontario Health (OH) eForms are an Ontario Health initiative that provides a digital form you complete and submit electronically. You can access eForms directly from a patient's chart using the built-in provincial integration service viewer. The eForms are automatically populated with the patient's and provider's information, saving you time.

eForms are available in English and in French. You can switch between languages by selecting the EN or FR button at the top of the eForms window.

📌Note: eForms are different than CHR’s forms. You can submit the eForms electronically to the Ontario Health instead of printing or faxing them and they are not automatically added to the patient's chart.

Steps

  1. Launch the eForms service from the CHR’s built-in viewer. For more information, refer to Logging into provincial integration services.

2. The Terms of Use - eForms agreement appears. Scroll to the bottom and click I Accept.

📌Note: This window opens each time you log in and you must accept to proceed.

3. The Dashboard opens. Here you see the list of previously submitted eForms. In the top right corner, you see the patient’s name and health card number.

4. Click Submit New Form to open a new eForm.

5. The form opens in the viewer with the patient demographics auto-populated. Practitioner information will also be auto-populated. Complete the form as you normally would.

📌Note: The Street Number field does not accept special characters. Remove any special characters before submitting.

📌Note: The form is not saved as you are filling it out. If you close the window, you will need to begin the process again.

6. When you are finished, type your name in the Signature of Practitioner field and select today’s date to sign the document.

7. Click Submit to send the form.

8. After the form is submitted, you can choose to download a PDF copy of the form to save in the patient's chart. Refer to Uploading patient files.
