You can quickly access eReports directly from a patient's chart, using the built-in provincial integration service viewer. You can view Cancer Care Ontario's Screening Activity Report (SAR) and the COVaxON Aggregate Primary Care Vaccination Report.
Cancer Care Ontario Screening Activity Report (SAR)
The SAR provides data for breast, cervical, and colorectal screening to help physicians improve their cancer screening rates and appropriate follow-up. It is available to primary care physicians who practice as part of a patient enrolment model. It is also available to physicians and nurses who practice in the Sioux Lookout Municipality and Sioux Lookout Zone (27 First Nation communities) who are not in a patient enrolment model.

COVaxON Aggregate Primary Care Vaccination Report
The COVaxON Vaccination Report is available only to family physicians practicing in a patient enrollment model (i.e. CCM, FHG, FHN, FHO, FHT). The report includes patients who have received COVID-19 vaccinations, along with the type and date of vaccination, and any symptoms immediately following vaccination.
Steps
Launch the eReport portal from 's built-in viewer. For more information, refer to Logging into provincial integration services.
From the main page, click the appropriate tab to view the SAR Dashboard or the COVaxON report.
