To have the ability to view CCM Alerts, you need to make sure that you have Embedded View section added to your Summary Screen.

Chronic Disease management alerts are based on six active problems: Diabetes, Hypertension, Coronary Artery Disease (CAD), Congestive Heart Failure, Asthma, and COPD. You can add these to a patient's Active Problems in their Medical History Section:

Once you have clicked into the Problem List you have the ability to add Active Problems. To add the problems, just click on the space beside Active Problems. It will bring up a list to search. All CCM problems are listed on top of the list:

The Embedded View will display the Chronic Care Management window which is located at the bottom of the Summary Screen. Here you will find all the categories the patient falls into by active problem, age, and sex. It will list the service and the date it was last completed. It will also let you know if the patient is compliant. 

This example below is a patient with Hypertension only:

This example below is a patient with Hypertension and Diabetes. The Lipids are also incompliant:
