If you want to send a referral, letter, or fax to an individual provider, professional or other person through the TELUS Collaborative Health Record (CHR), they must be added to your clinic's "address book" as a contact.

Before you can add a contact, you must add the contact's facility. You then add the contact from the facility. A facility can have an unlimited number of contacts associated with it. If a contact works out of multiple facilities, you must re-add the contact from each facility. See Adding and modifying facilities.

πŸ’‘ Tip: If you're adding a provider or specialist who works out of a single-provider clinic, you can create a facility with the provider's name (with the facility Type set to Physician), and then add the provider as a contact from the facility.

When you add contacts, every user in your clinic has access to them.

Steps

1. From the main menu, click Contacts. A list of facilities displays.

2. In the top-right corner, click the Search icon and then search for and select the facility for which you want to add or edit a contact.

πŸ’‘ Tip: If you're editing a contact, but you don't know their facility name, do the following. In the list beside the field, select Contact Name, and then search for the contact's name. When you select the contact, their facility window opens.

3. If the contact's facility doesn't exist, click New Facility and add the facility. See Adding and modifying facilities.

4. In the facility window, in the Contacts area, perform one of the following actions:

  • To edit a contact, in the list of contacts, click the contact.

  • To add a contact, click New Contact.

5. In the contact window, using the following table, add or update the contact's information.

πŸ“Œ Note: You can fill in as much or as little of the information you need. If you send faxes to this contact, make sure you add their fax number.

Field

Description

Title

Select the contact's title. For example, if they are a physician, select Dr.

First Name

Enter the contact's first name.

Last Name

Enter the contact's last name.

Occupation

Select the contact's occupation or role.

πŸ“Œ Note: If you select Medical Doctor (M.D) or Resident, the Medical Specialty field appears.

Medical Specialty

Select the provider's specialty.

πŸ“Œ Note: This field is available only if you select Medical Doctor (M.D) or Resident in the Occupation field.

πŸ’‘ Tip: By selecting a specialty, you'll be able to search for this contact by specialty when referring patients.

Billing Number

If this is a provider with a ministry billing number, enter the number.

πŸ“Œ Note: If this is a specialist to whom you refer patients for many provinces, this number is required.

License Number

If this is a provider or professional with a license number, enter the number.

Jurisdiction

Select the contact's province.

Email

Enter the contact's email address.

Phone

If the contact's phone number is different from their facility's phone number, enter their phone number, including the area code.

πŸ’‘ Tip: You can type the phone number without adding any dashes or brackets. The CHR auto-formats the phone number as soon as you save your changes.

Phone Ext

If the contact has a phone extension number, type the number here.

Fax

If the contact's fax number is different from their facility's fax number, enter their fax number, including the area code.

Comments

Type any important notes regarding the contact's referral processes, availability, etc. For example, "This office is accepting patients".

6. Click Save. You return to the facility window.
