When you add a new patient, much of the demographic data is pulled in from the Client Registry. Then when demographic updates are made in other systems, these changes will automatically flow into the patient chart in CHR. Similarly, certain demographic changes you make in the CHR will be sent back to update the Client Registry.

📌 Note: You can see when a patient chart is synced with the Client Registry when searching for a patient (a green check icon appears under the patient name), and

in the patient's chart header (a green Synced banner appears in the patient header). For more information, refer to Syncing and unsyncing with the Client Registry.

Not every demographic field is synchronized between the Client Registry and the CHR. Review the table below to understand which information can come from the Client Registry and which can be sent from the CHR to the Client Registry.

📌 Note: Email address is not part of the Client Registry updates.

Fields updated in the CHR from the Client Registry

  • First name

  • Middle name

  • Last name

  • Date of birth

  • Sex

  • Cell phone

  • Home phone

  • Office phone

  • Identification (health number)

(Patient chart > dashboard view > Update Information. Also in the Patient Data > Demographics section of the chart).

📌 Note: The phone numbers will only be pulled from the Client Registry when the patient chart is first added to the CHR. They will not be updated from the Client Registry after that.

  • Medicare eligibility (click the gear icon in the Identifications field)

  • Mailing address

  • Home address

(Patient Data > Demographics > Addresses)

  • Date of death

(Patient Data > Demographics > Status)

📌 Note: Any changes you make to patient demographics in the CHR are visible to other offices immediately, since the patient demographic information is shared across the entire CHR domain.
