TELUS Health provides a wide variety of Qnaires through a national content repository. Depending on your needs, you can request existing Qnaires, build your own, or hire TELUS Health to build one.
Before requesting a Qnaire, browse the CHR Qnaire Library (PDF) to see if the Qnaire already exists. If it does, contact the TELUS Health support team through the support chat (refer to Contact us) and provide the name of the Qnaire you want added to your CHR.
