🎞️ Video: How to build Qnaires

TELUS Health has many Qnaires available in a national content repository. If you are already live and want these Qnaires added to your CHR account, contact the TELUS Health support team through the in-app support bubble. See Contact us.

We do not typically build clinic-specific Qnaires and therefore recommend you create your own. If you do not have time to build your own, contact the TELUS Health support team. Charges will apply for the creation of clinic-specific Qnaires.

If you are in the implementation phase, your Project Manager will work with you to offer any of our Qnaires available in our national content repository, or they will help you build clinic-specific Qnaires for your practice.

⚠️ Important: To build or modify Qnaires, you require the Edit Qnaires permission.

πŸ’‘ Tip: When building templates, you may receive a message that your CHR session has timed out. This happens because the CHR session timeout does not apply to pop-up windows. To fix this, simply change your timeout setting to eight hours while building your templates. When you're done creating your templates, switch the setting back to something more reasonable. See Setting your personal timeout duration.

If you have French enabled in your CHR, you can create Qnaires in both French and English.

Steps

  1. From the main menu, click Qnaires. A list of all published and unpublished Qnaires in your CHR account appears.
    πŸ“Œ Note: If you have French enabled in your CHR, the list of Qnaires appears in your current language. For example, if you're logged in with French and a Qnaire has a French translation, the French Qnaire name shows in the list.

  2. To create a Qnaire, at the top-right corner, click New Qnaire.
    πŸ“Œ Note: To modify an existing Qnaire, select a Qnaire from the list.


    The Qnaire builder opens in a new page, with the General Information section displayed.

  3. Add or modify the Qnaire name, description, and image. See Adding or modifying a Qnaire name, description, and icon.
    β€‹πŸ“Œ Note: For optimum image results, use a 180x180 pixels JPG, PNG or PDF image.
    πŸ’‘ Tip: If you have French enabled in your CHR, select EN or FR at the top-right to build the Qnaire for that language.

  4. Click Next Step.
    πŸ“Œ Note: Alternatively, at the top of the page, click the Questions tab to move to the next section.

  5. The Questions section appears.

  6. Add or modify questions in the Qnaire. For more information, see Adding questions to Qnaires.

    πŸ“Œ Note: Once you have added questions, you can add section breaks with or without headers to group questions. See Grouping questions in Qnaires and Adding headings to grouped questions in Qnaires.

    πŸ’‘ Tip: Adding or deleting questions or section breaks applies to both languages if you have French enabled in your CHR. When you add a question, it is added to both languages as you've entered it; switch to the other language to provide the translation for that question.

  7. To view how the Qnaire appears to those completing it, click Preview. If you have French enabled in your CHR, the preview appears in the language you currently have selected in the Qnaire.
    πŸ“Œ Note: To return to the Qnaire builder, click outside the preview Qnaire screen.

  8. After adding or modifying questions, click Next Step.
    β€‹βš οΈ Important: Ensure that there are no blank questions in your Qnaire before you click Next Step.


    The Publish section appears.

  9. Format the Publishing Checklist specific to your Qnaire and publish your Qnaire. See Publishing your Qnaires for more information. If you have French enabled in your CHR and you created a bilingual Qnaire, the Qnaire is published in both languages.
