Adding headings to grouped questions in Qnaires

Section headers

You can organise your Qnaires with section breaks. See Grouping questions in Qnaires. You can add text (section title) with or without an additional description, image or video to accompany the section break (grouped questions).

📌 Note: You can choose to view or hide these section titles in the Qnaire response sheets.

Steps

1. From the Questions page within a new or existing Qnaire, click +New Question. See Creating Qnaires for information on how to navigate to the Questions page.

A New Content box with all the possible Qnaire question types appears.

2. Under Choose Type, click Section.

A Section Header window appears.

3. Complete the applicable fields using the table below:

Enter Section Title

Type the section header in this field.

Question Subtext (Optional)

Add any additional information related to the section.

  1. Click the empty field.

    An Edit Description window appears.

  2. Type text in the blank field.
    💡 Tip: You can format your text using the formatting bar at the top of the message body.

  3. To add an image in this field, click the image icon in the formatting bar. For more information, see Adding images to Qnaires.

  4. To add a video in this field, click the video icon in the formatting bar. For more information, see Adding videos to Qnaires.

Question Subtext (optional)

Visible on Review?

No is selected by default.

Select Yes if you want to see the section title in your patient's answer sheet.

Type a section header that you want to see on the patient's answer sheet if it is different from the section title above.

💡Tip: If you have natural language configured in a Qnaire, i.e to pull a patient response into your encounter notes, and you want to add the section header to the encounter notes, you must type the heading in the Natural Language Conversion field of the question that appears after the added section question. See Setting up Natural Language.
