When you add a patient to the CHR, you must include an identification. This is generally the patient's provincial health number, but can also be other forms of identification such as an RCMP number, a treaty card number, or a third-party insurance number. You can add multiple identifications.

Steps

1. Open the demographics window for a patient.

  • For a new patient, from the main menu, click Patients > New Patient. (For more information, refer to Adding patients).

  • For an existing patient, open their chart. (For more information, refer to Searching for patients). From the Dashboard view, click Update Information or, in the Identifications widget, click Manage Identifications.

2. In the Identifications section, click +New Identification. To edit an existing identification, click the gear icon beside the identification.

3. Select the type of identification from the list. For example, select the appropriate province for a provincial health number.

📌 Note: If the patient has an existing identification, that type is grey in the list; you can add only one identification for each type.

💡 Tip: If there is an identification you frequently use but it is not in the list, you can add it in Settings. Refer to Adding additional patient identifications.

4. Type the identification in the Identification Value field. If the identification is not in the correct format for the selected type (for example, the wrong number of digits for a provincial health number), the field turns red when you move away from it.

⚠️ Important: Do not include spaces, hyphens, or slashes. If you include other characters or spaces, in some provinces the health number may be seen as invalid by the billing provider (e.g. MDBilling in Ontario).

5. Depending on the type of identification selected, you can specify additional information such as the Version Code for an Ontario health number or the Registration Number for Manitoba.

6. Optionally enter the Plan Start Date and Plan End Date. This is useful when a patient recently moved from another province and is still covered under the old card until the new card becomes effective. You can record the identification for both provinces, with their appropriate start and end dates. Type the dates or click the calendar icon to select a date.

For a Quebec health number, optionally enter the Expiry Date.

7. If the patient has multiple identifications, you can select which ones appear on prescriptions. Select the checkbox to Include this identification on this patient's prescriptions.

You can select up to three identifications to include on prescriptions. If there are three already selected, the checkbox is greyed out and when you hover your cursor over the setting a tooltip displays indicating that you can add only three.

8. Click Okay.

The identification is added to the patient demographics. If there is only one identification, it is automatically marked as the Primary identification. If there are multiple identifications, click Primary next to the one you want to mark as the main identification.

📌 Note: The identification shown at the top of the patient's chart (in the header) is the primary identification.

If you specified that the identification should be included on prescriptions, an Rx icon appears next to it. If you did not specify an identification to be used on a prescription, the primary identification is used. If a primary identification is not set, the first identification in the list is used.

9. To delete an identification, click the trash can icon next to it and then click Okay to confirm.
