When a patient completes a Qnaire with Medical History, Family History, Surgical
History, or Obstetrical History questions, the CHR automatically links their
responses to the Health Profile in their chart. You must review and confirm these responses before the CHR adds them to the patient's chart.
๐Note: To skip the manual review and enable automatic Health Profile updates, turn on patient data automatic updates when you publish the Qnaire. Refer to Linking Qnaire responses to patient data and Publishing Qnaires for more information.
Steps
1. Open a patient's chart.
2. A prompt notifies you that a New Qnaire Response is available for the patient.
3. Click Review now.
๐ Note: If you click Skip, the prompt will reappear each time you refresh or reopen this patient's chart until you confirm all new items.
4. The Health Profile window opens. An indicator displays the number of Health Profile items with new information.
5. Click the applicable health profile section to expand and review the items in bold that require confirmation.
๐ Note: Items already entered in the CHR or confirmed from previous Qnaires
do not require confirmation.
6. To edit a patient response, click it, make your changes, and click Save and Confirm to dismiss the notification.
๐กTip: Patient responses display as free text. Click a response to edit the item and select a diagnosis or procedure from the list.
7. To confirm individual items, click the checkmark icon.
8. If there are three or more new items, click Confirm All.
9. After you confirm all items, the indicators disappear and you cannot reverse the confirmation.
๐ Note: Health profile items will not appear in the Summary view until you approve them.
10. To remove an item, click the trash icon to the right of an item.
๐ Note: You can click the history icon to view who added the item (such as the patient) and track your changes.
