Numerical questions allow patients to enter a number as a response to a specific question. For example, a patient is asked to enter their weight.

You can score the answers to numerical questions based on the patient response. When a patient enters 100 in a numerical question it can be used as the score. For example, a clinic uses a Qnaire to calculate infusions based on patient weight and medication dosages.

Steps

  1. From the Questions page within a new or existing Qnaire, click +New Question. For more information about opening the Questions page, see Creating or modifying Qnaires.
    A New Content box with all the possible question types appears.

  2. Click Numerical.

  3. Click New Question and fill out the fields using the following table:

Enter Question

Type a question to ask your patients.

Question Subtext (optional)

Type any additional information related to the question.

This appears under the main question.

Question Label (optional)

To change the way the question appears in the Qnaire responses, type a new Label. For example:

Question: How much do you weigh? Question label: Weight.

The question appears on the answer sheet in patient charts as Weight.

Is Optional

Select Yes to make the question optional to complete or No to make it mandatory.

Use input value for scoring

Select Yes if you want to use the value entered by patients in custom scoring. See Configuring custom scores in Qnaires.

Select No if you want to add score modifiers to value ranges. See Adding score modifiers to questions.

Singular Unit

Type the single unit you want displayed in the Qnaire and in the encounter.

Plural Unit

Type the plural unit you want displayed in the Qnaire and in the encounter.

Unit format

Type the format of the unit. For example: 0 or 0.00.

Ranges

Add the minimum and maximum value the patient is allowed to enter.

This appears under the main question.

Qnaire Variable Name and Exportable Name (optional)

This is used for exporting purposes (simple exports / data slices).

Type a name/ code you want to see when exporting data.

Natural Language Conversion (optional)

Allows you to convert a patient's response into a sentence that appears in an encounter note under the History section.

Numerical question types pull the number entered by the patient as the answer.

Update Patient Data

Select, if you want to update patient data in the patient chart. See Linking Qnaire responses to patient data for more information.

📌 Note: You can modify the patient data by clicking Edit.

📌 Note:

  • If you selected Yes for Use input value for scoring, the fields (Enter option, Value Ranges and Natural Language Conversion) under the question do not apply.

  • If you selected No for Use input value for scoring, the fields Enter option and Natural Language Conversion under the question do not apply. The fields Score Modifier and Value Ranges are used to add score modifiers when configuring custom scores. See Adding score modifiers to questions.

4. Click Next Step to move to the Publish page. See Publishing your Qnaires.
