You can group common Qnaires you use into folders, to avoid scrolling through a long list of clinic Qnaires.

For example, if you work in a multi-specialty clinic with many Qnaires that do not apply to your field, you can create a folder to add only those Qnaires that you commonly use. Or, if you work in a clinic with many Qnaires per condition/group (e.g. Intake Qnaires), you can create folders for each group. Having relevant Qnaires organised in folders makes it easier to find and access them.

📌 Note: You can create Folders and organise Qnaires and templates for any user, if you have the Folders permissions. You only have access to folders assigned to you when using the CHR.


  1. To create your own folders, click your avatar image and click Contents.

  2. To create folders for other users, from the main menu, click Settings > Folders and search for and select a name.
    💡 Tip: To search for a name, use the Google Chrome search function (Ctrl/Cmd F) to open your web browser's search functionality.

  3. From the window that appears, click +Add Folder.
    A Folder Content window opens.

  4. In the Name text box, add the folder's name, for example, Intake Qnaires.

  5. In the Description text box, type any additional information about the folder.

  6. Click +Add Content.

  7. From the new window that appears, under the Qnaires tab, search and select one Qnaire at a time to add to your folder.
    💡 Tip: To search for a Qnaire, use the Google Chrome search function (Ctrl/Cmd F) to open your web browser's search functionality.

    📌 Note: You can also add Encounter Templates, Form Templates, and Letter Templates to your folder. See Organise your templates into folders.

  8. To remove a Qnaire from your folder, hover over the name and click the x that appears.

  9. Click Apply.

  10. To remove the folder, click the trash icon. A confirmation window appears. Click Okay if you are sure to delete it.

  11. To add more items to the folder, select it and follow steps 6 to 9 above.

  12. Click Close.

When you want to select a Qnaire in the CHR, the Select Folder window opens, and you can either choose a Qnaire from your newly created folder or all clinic Qnaires.
