Setting up recurring Qnaires

You can send Qnaires to your patients on a recurring basis at a pre-defined interval. For example, you can create a schedule for the CHR to automatically send an Anxiety Self-Assessment Qnaire to your patient every week to track their progress over some time. You can configure who receives the patient's responses.

If, after a certain period, the patient did not complete the Qnaire, the system can automatically send them a reminder. You can also notify a user if the patient did not complete the sent Qnaire. The notification appears in the selected users' CHR Inbox.

When you send a bilingual Qnaire, the Qnaire opens in the same language as the Qnaire notification (the patient’s primary or secondary language, or the account language if the patient’s preferred language is not specified). The patient can change the language before completing the Qnaire.

📌 Note: If a patient switches languages after responding to some questions, they are warned they will lose their responses.

Steps

1. From the patient chart, click Start/Open > Scheduled Qnaires.

2. Under Recurring Questionnaires, click + New Schedule.
A New Recurring Questionnaire window opens.

3. Complete the fields using the table below:

Field

Description

Delivery Routine

Select Daily. A new field appears. Select the interval, and time the CHR sends the Qnaires.

Select Weekly. A new field appears. Select the interval, the day of the week and the time the CHR sends the Qnaires.

Select Monthly. A new field appears. Select the day of the month, the interval, and time the CHR sends the Qnaires.

You can enter a date when you want to end the recurring Qnaire(s). In the field that appears when you select the delivery routine, next to Until, click the calendar icon and select a date.

Delivery Method

Select the method used to send the Qnaires to patients.

The options are Patient Preference (according to their demographic information), Both, SMS and Email.

Practitioner

Select the provider to receive the Qnaire responses in their inbox.

4. Click +Add to search for and select the Qnaires.

5. If you want to send the patient a reminder when they have not completed the Qnaire:
a. Click the Advanced tab.
b. Under Patient Reminder (optional), input a time period. Once the time period expires and the patient hasn’t completed the Qnaire, the CHR automatically sends a reminder to the patient.

📌 Note: In the first field, you can enter any number, and in the second field, select Minutes(s), Hour(s), or Day(s).

c. To delete the reminder, click the trash can icon.

6. To set up an alert for yourself if the patient has not completed the scheduled Qnaire:

a. Click the Advanced tab.

b. Under Internal Clinic Reminder (optional), input a time period. Once the time period expires and the patient hasn’t completed the Qnaire, the selected user automatically receives an alert in their CHR inbox.

📌 Note: In the first field, you can enter any number and in the second field, select Minutes(s), Hour(s), or Day(s).

c. Select a provider from the Practitioner list.

d. To delete the alert, click the trash can icon.

7. Click Save.

The saved recurring Qnaire appears under the Recurring Questionnaires section.

📌 Notes:

  • To customize the view, click the gear icon.

  • To pause the sending of the recurring Qnaire, in the Action column, click the pause icon.

  • To delete the recurring Qnaire, in the Action column, click the trash can icon.
