You use date question types when you want your patients to enter a date. You can ask patients to enter a specific date (i.e. select the year, month and day) or rather an estimate date (i.e select the year or month).
📌 Note: The choice option I don't remember exactly cannot be removed from date question type.
Steps
1. From the main menu, click Qnaires.
2. Start a new Qnaire or select an existing one. See Creating and modifying Qnaires.
3. Click the Questions tab and in the top-left corner, click New Question.
📌Note: At the bottom of your Qnaire, you can also click +New Question.
A New Content window with the possible question types appears.
4. Under Choose Type, click Date.
A new window opens.
5. Complete the applicable sections using the below table:
Question | Description |
Date Question Text | Type your question. |
Question Subtext (optional) | Type any additional information related to the question. This appears under the question. |
Question Label (optional) | To change the way the question appears (shortened) in the Qnaire responses, type the label.
For example: Question: When was the last time you had surgery? Question label: Surgery Date. The question appears on the answer sheet in the patient's chart as Surgery Date. |
Date Fields Requested | Select Year, Month, and Date if you want the patient to enter the exact date. |
Allow future dates | Select Yes to allow the patient to enter future dates. In the Future year limit field, enter the cut-off date in years.
Select No if you want patients to only enter past dates. |
Optional | The default is No, making the question mandatory to complete. Select Yes to make the question optional to complete. |
Qnaire Variable Name and Exportable Name | This is used for exporting purposes (simple exports / data slices). Type a name / code you want to see when exporting data. |
Natural Language Conversation | Allows you to convert a patient's response into a sentence that appears in an encounter note under the History section. Date question types pull the number entered by the patient as the answer. For more information, see Setting up natural language.
📌 Note: If patients do not enter the complete date from the provided options, the answer on the response sheet appears as “No answer”. |
Update Patient Data | Select if you want to update patient data in the patient chart. See Linking Qnaire responses to patient data for more information.
📌 Note: You can modify the patient data by clicking Edit. |
💡Tips:
To move the question to a different position in the Qnaire, in the left side panel, click and drag the question to the suitable position.
To delete the question, in the top-right corner click X.
To duplicate a question click the copy icon at the bottom-right corner of the question.
6. To add more questions, click +New Question.
7. To publish the Qnaire, click Next Step.See Publishing your Qnaires.
📌 Note: If you edited an existing Qnaire and you are done, close the Qnaire builder window. The Qnaire builder automatically saves your changes.
