🎞️ View video tutorial: Recording vitals and measurements from encounters
When you record encounter notes, in the Examination area, you can record the patient's vitals and measurements without having to open the Patient Data area of the chart.
📌 Note: When you save the encounter, the vitals and measurements are added to the Patient Data area of the patient's chart. For children, the CHR automatically updates their growth chart in the patient dashboard section of their chart.
Steps
1. Start an encounter for the patient. See Starting an encounter.
2. At the top of the Examination section, click Add Vitals. The vitals and measurements entry fields display in a table format.
💡 Tip: You can set your encounter view to display the patients' previously recorded vitals and measurements in the Examination area. See Displaying a patient's previous vitals and measurements in encounters.
3. In one or more of the entry fields, type the patient's values. For example, you can enter height, weight, systolic and diastolic blood pressure, heart rate, respiratory rate, temperature and O2 saturation.
💡 Tips:
When you enter both the height and weight, the CHR calculates the BMI.
You can customize what vitals or measurement columns display in the entry table. See Customizing what vitals and measurements entry columns display in encounters.
To view vitals in a graph format, click the respective vital heading. See Graphing vitals and measurements.
To delete vitals, you must delete the entire row. If you only want to delete one vital you must delete it from Patient Data > Vitals.
4. To add multiple values for the same measurement, for example historical records, click + Add Vitals again and type the values in the new entry line. Each row appears as its own entry in the patient data points History. The Time column is used to sort the entries in chronological order.
