Configuring your form and cover page templates to autofill

The TELUS Collaborative Health Record (CHR) forms are electronic versions of the standard forms your clinic uses. You use these electronic forms while in a patient's chart. See Using electronic forms.

Autofill configuration enables you to pull information from the patient chart, your location, or provider information to pre-populate your form or cover page with the required data. This saves time and streamlines your workflow.

Text box or checkbox variables in your template are mapped to a specific data point in the CHR. When using the form or cover page, the mapped data is pulled into the form or cover page.

For example, a text box variable is mapped to the patient's last name. When using the form or cover page, the patient's last name auto-populates. If the field is empty in the patient's chart, nothing will populate in the variable.

You can map the following fields:

Patient Information

  • Patient demographics (such as name, sex or contacts).

  • Identification to pull in health card numbers or other IDs.

  • Social History (from Patient Data).

  • Vitals (such as weight, blood pressure or head circumference).

  • Latest lab results pulled from electronic labs in the patient chart.

  • Medical History (such as Problem list).

  • Preventive care, if configured (such as FIT test, Mammogram, Covid-19).

  • CDM, if configured (such as Hypertension, Exercise reviewed or Asthma).

  • Configured forms, such as Rourke.

  • Qnaire Scoring.

  • Social History and Obstetrical care information (from Health Profile data).

Other Information

  • Date (selected, created or appointment).

  • User (can be current or selected user, and can pull name, billing number, etc).

  • Location (such as name, address or fax number).

  • Appointment (such as note, date or service).

  • Allergies

  • Recipient (such as name, fax or address).

  • CC Recipient (such as name, fax or address).

  • Lab Identifier (distributor or identifier).

  • Care Team (such as name, title or billing codes).

  • Form Data (must be configured first in Data Templates).

  • Referral Data (must be configured first in Data Templates).

  • Lab Result (pulls from the lab results in your patient charts. If you have lab mapping set up, it will pull the naming from there (see Mapping lab synonyms).

💡Tips:

  • To pull patient data into a form or cover page using the variables, when opening the form or cover page, ensure that you have Patient Data and Health Profile selected in the Autofill Templates window.

  • The fields Selected user, Selected date, Location, Appointment, Recipient, CC Recipient, and so on pull from your selection in the Autofill window.

Before you can configure your form to auto fill, you must first create your template. See Creating cover page templates and Creating form templates.

Steps

1. Click the area in your form or cover page template where you want to add the variable. A new window appears.

2. Click Check Box or Text Box. A new window appears.

4. Under Auto Fill Configuration, choose Patient Data, Referral Data, Form Data, Lab Result, or Health Profile.

5. Click the gear icon to configure what field the checkmark is mapped to. The Set Data Variable window opens.

⚠️Important: You must make your selections from top-down because the options are linked to each other.

6. For Patient Data, Referral Data, or Form Data,

a. From the Category list, select a field (such as Demographics, Vitals, User).

b. From the Data Property list, select a field (such as Preferred Notification Method, Blood Pressure, Selected User).

c. Leave Ignore Blank Values as False.

d. From the Visible Data list, select a field (such as Family Name, Systolic, Billing Number).

For example:

📌 Note: Depending on your selection, you may have other fields to complete. For example, a Date category has an additional field for Custom Format. From the drop-down list select from the options or type YYYY/MM/DD, MM/DD/YYYY etc.

7. For Lab Result,

a. Under Lab Result, select a lab result. The list pulls from all the labs that exist in your patient's charts. If you have lab mapping set up, it will pull the naming from there (see Mapping lab synonyms).

b. Under Format, select Simple or Advanced.

  • Simple: From the list, select what part of the result you want to appear in the template.

  • Advanced: Type the exact format you want to appear in the template.

8. For Health Profile, under Health Profile Section Category, select Social history record or Obstetrical care record and then the specific variable and visible data for that category.

📌 Note: The Health Profile data that pulls into a form is the most recent value. For Social History variables, if multiple entries exist for the same variable, the information is not auto-populated since the CHR cannot determine the most recent entry. For example, if you’ve recorded recreational drug use for more than one type of drug, the drug use information is not auto-populated.

9. Click Save.

💡 Tips:

  • For health card numbers, use Identification > Value.

  • The Manitoba Health Reg or Ontario Version Code is Identification > Primary > Extra Value.

  • If Default address does not pull, under Visible Data, use Street, City and so on and create multiple variables until you have the full address.

  • Selected Date defaults to today's date. Click to change it on the form. All Selected Date variables on a form are connected and populate the same date. If you need two different dates, use Selected Date and either Created date or Appointment date.

  • For some variables, you can use HTML in the Format field.

  • If a variable is not pulling data into your form, ensure that the data exists in the CHR. For example, you include a variable for Drug Allergies, however, the patient has none.
