The TELUS Collaborative Health Record (CHR) forms are electronic versions of the standard forms your clinic uses. You use these electronic forms while in a patient's chart. See Using electronic forms.

You can insert drop-down (or selectable) lists to easily choose from a configured list of data items in your forms. This streamlines your workflow if you have a set of options to choose from when filling out a form field, such as a set of symptoms or conditions.

📌 Note: Form data templates are the individual collections of data, or the actual list you insert into your form template. They are created as part of data templates, this feature must be configured by the TELUS CHR Support account. Contact the TELUS Health support team through the support chat for more information. Once the form data template is created, anyone with Template permissions can add it to the form template.

If you are logged in with the TELUS CHR Support account follow the below steps to create your Form Data Template (the drop-down list).

Steps to create a drop-down list (TELUS CHR Support account)

  1. From the main menu, click Settings > Data Templates.

  2. Click the Form tab.

  3. Click +add. The New Category window opens.

    📌 Note: To edit an existing template, click the associated category tab.

  4. Type a name for your form template category.

  5. Click Save. A new tab is added with your category.

  6. Ensure that your new category is highlighted (blue) and click + Add Property. The Edit Property window opens.

  7. In the blank field, type a name for your property.

  8. Click + Add field. Each field is a drop-down list. When adding to the form template this is the Visible Data.

  9. Select Multi Select or Single Select from the pop-up.

  10. In the right-side panel Field Name field, type a name for your drop-down list.

  11. Leave Display Type as List.

  12. Add each drop-down item to the Values field. Hit Enter after each item so that each option is on its own line.

Steps to add a list to a form template

Follow the steps below to add your drop-down list to your form template. Before you can add drop-down lists to your form template, you must first create your template. See Creating form templates.

  1. Click the area in your form template where you want to add a drop-down list. A window appears.

  2. Click Text Box. The Update Text Box window opens.

  3. Apply any configuration needed to your text box (see Adding text boxes to form and cover page templates).

  4. Under Auto Fill Configuration, select Form Data, and click the gear icon. The Set Form Data Variable window opens.

  5. Select the Category, Data Property and Visible Data, as you configured above for your Form Data Template.

  6. Click Save.
