In your CHR, you can graph vitals and measurements from an encounter and from the patient Summary.

Each vital or measurement has its own graph except blood pressure where systolic and diastolic readings appear on the same graph. The latest 5 entries per vital appear in one graph.

๐Ÿ“Œ Note: Even if you do not enter a specific vital or measurement in the encounter, you can still graph the last five readings of a vital you previously documented.

Steps

1. If you have an encounter open, (see Starting an encounter), scroll to the Examination section.

๐Ÿ“Œ Note: If you do not see the Examination section, select the gear icon at the bottom-right corner and clear the Physical exam checkbox. Refer to Customising your encounter settings.

๐Ÿ’ก Tip: Depending on your encounter settings, you can configure your vitals view to display the patient's previously recorded (maximum five) vitals and measurements in the Examination area. Refer to Displaying a patient's previous vitals and measurements in encounters.

Alternatively, in the patient Summary view, expand the Patient Data > Vitals section.


2. To graph the last entries (maximum 5) of a specific vital, click the name of the vital.

๐Ÿ“Œ Note: In an encounter, clicking either SBP or DBP will graph both blood pressure values on one graph.

The graph and table appear in a window with the patient's full name and the type of vital or measurement as the heading.
โ€‹

๐Ÿ“Œ Note: You must save any newly recorded vitals before they can be shown in the graph. See Recording patient vitals and measurements in encounters.

3. Review the trending values in your graph and table:

  • The last five entries for the specific vital or measurement are displayed in the graph and table.

  • The date displayed is when the vital was first entered.

  • The X-axis displays the date the vital was entered and the Y-axis displays the vital units.

  • For blood pressure, triangle icons show the systolic (green) and diastolic (blue) values.

  • For all other vitals and measurements, a small green dot icon is used to present the values.

  • The readings taken at different times have lines connecting them to show trends clearly.

  • When you hover-over a dot or a triangle, the date, vital name and its reading are displayed.

4. To close the graph, click X in the top-right corner.
