Viewing Qnaire responses

You can access Qnaire responses from the Inbox, Encounter or directly from a patient's chart. You can view trends and compare responses in each of these areas.

Viewing the Qnaire responses is the same no matter how you open it.

Steps

  1. Open the Qnaire response from the Inbox, Encounter or directly from the Qnaire Responses section of the patient chart.

Inbox

When a patient completes a Qnaire, it goes into the assigned provider’s inbox. This lets you stay on top of patients' responses and review them as soon as possible. In the Inbox, Qnaires are identified with a clipboard icon.

You can view the Qnaire response from the Inbox, or click Open at the top-right corner to open the Qnaire in the Qnaire Responses section of the patient chart.

Encounter

To view a Qnaire response from an encounter, it must first be attached to the Qnaires section of the Encounter note. For more information, refer to Attaching Qnaire responses to encounters.

To open the Qnaire response, click the Qnaire name from the Finished Qnaires list; it opens in a new panel on the right-hand side. This allows you to view the Qnaire response and your encounter note at the same time.

Qnaire Responses section of the patient chart

The most common way to view Qnaires is from the patient’s chart, as it allows you to view all historical Qnaires for the patient.

Steps

  1. From the patient chart, click Start/Open > Qnaire Responses. The Qnaire Responses section opens.

    All of the patient’s Qnaire responses are listed. The Qnaire name, created date and score show in the table.

    📌 Note: A coloured square beside the score indicates the responses rating based on the total score.

  2. To view the Qnaire responses grouped by Qnaire and in a table comparing the Score over time, at the top-right corner, click Grid.

  3. From the Grid view, click the Score, or from the List view, click the Qnaire Name. The Qnaire response opens in a new window.

The Qnaire response is divided into different sections: the heading, Answers, Summary and Note.

2. The following information appears in the heading.

  • The Qnaire name.

  • The date and time the patient completed the Qnaire and how long it took them.

  • If the Qnaire is signed, the date and who signed the Qnaire.

  • If the Qnaire has scoring configured, a graph with total scores appears comparing the current score with previous scores from the same Qnaire. Hover over a point to see additional details, and click it to open that specific Qnaire response.

  • Total Score.

You can perform the following actions in the heading:

  • Does not include the Summary section.

  • If the Qnaire is signed, the provider's signature and a timestamp are included.

PDF (with Text Output)

Click to generate a PDF of the Qnaire response along with patient information.

The natural language from the Summary section is included.

📌 Note: If the Qnaire is signed the provider's signature and a timestamp are included.

Previous Answer Sheets

Click to view all responses for the same Qnaire.

A Previous Qnaire Responses window opens. Click a response to open it.

💡 Tip: The Qnaire response you are currently viewing is highlighted in grey.

3. The Answers section shows the Qnaire questions along with the patient's answers. On the right-hand side, click to view the answers in Standard View or Compare View.

If it is the first time the patient has completed this type of Qnaire, the Standard view is shown by default. Standard view shows only the current Qnaire responses.

📌 Note: A coloured square beside the score indicates the answers rating. For more information, refer to Scoring Qnaires.

If the patient has previously completed the same Qnaire, the Compare View is shown by default. Compare View shows the current Qnaire response along with the previous response.

📌 Note: Arrows beside the response indicate if the score has increased or decreased and a line indicates that the score has remained the same from the previous Qnaire response. The red, green or bold text shows the answers rating.

In the Answers section, you can:

📌 Note: The Qnaire must be unsigned.

Click to open the Trend of Answer window and view a comparative graph of all responses to that question.

Only available for picture question types.

The patient provided image is not automatically saved to the patient’s chart. Click the save icon to save the image to the Patient Files section.

4. Depending on the configuration of the Qnaire, the following sections appear:

Note

This note appears in the generated Qnaire response PDF and is saved in the Qnaire response.

For example, the note section can be used to explain scoring or trends.

📌 Note: When the Qnaire is signed, it cannot be modified.

Summary

Only shows if the Qnaire has natural language.

The Summary section shows any configured natural language. To easily copy all the text, click Copy this paragraph.

📌 Note: For natural language conversion outputs, you must configure the Natural Language Conversion field of your Qnaires.

5. From the grey bar at the top, you can do the following actions:

Click to return to the main Qnaire section.

Sign

Click to sign off a Qnaire response.

📌 Notes:

  • If the Qnaire is part of an encounter note, it is automatically signed when the encounter note is signed.

  • This action cannot be undone.

Delete

Click to permanently delete a Qnaire response.

📌 Note: You can't delete a signed Qnaire.
