You can easily include patient completed Qnaires as part of an encounter note. Adding a Qnaire to an encounter note ensures that the information provided in the Qnaire is associated with that specific visit. You can also easily click the Qnaire to view the patient's responses from within the encounter.
If the Qnaire is configured with Natural Language, the text pulls into the encounter note, allowing you to pre-populate your encounter note with patient reported information.
📌 Notes:
You can associate Qnaires with an appointment type or presenting issue. As well you can configure pre-visit Qnaires. These Qnaires are automatically attached to the associated encounter note.
You can configure any Qnaires completed today to automatically be attached to your encounter note. This is helpful if you have patients complete Qnaires in the waiting room. For more information, refer to Customizing your encounter settings.
If you sent a bilingual Qnaire, the Qnaire responses display in the language they were completed in.
Steps
Start, or open an encounter. Refer to Starting an encounter.
In the top-right corner of the Qnaires section, click + Attach a Qnaire Response. The Select Qnaire Responses window appears and shows all Qnaires completed by the patient.
Select the Qnaire responses you want to attach and click Add. The Qnaires are attached to the Finished Qnaires section of the encounter.
💡 Tip: You can select multiple Qnaire responses.
📌 Note: If your Qnaires contain natural language, the Auto-Populate Encounter window appears.
If you don't want to add the content to your encounter note, in the Attach Paragraph to list, choose None.
To add the content, in the Attach Paragraph to list, select History, Examination or Assessment and Plan.
Click to add the content to the Bottom or Top of existing data in that section.
Click Attach.
💡 Tip: If you always attach natural language in the same way, you can configure a default section and placement. For more information, refer to Customizing your encounter settings.
Complete the rest of your encounter as per usual. Refer to Recording encounter notes.
To remove a Qnaire response from the encounter, click the remove icon. The Qnaire remains in the Qnaire section of the patient chart.
