To proactively flag high-risk patients, you can create customized provider alerts received via email or SMS based on patients' Qnaire responses.

For example, you want to receive a notification if a patient has suicidal thoughts when responding to specific questions in the Depression Self-Assessment Qnaire.

You can configure these alerts in multi-select, radio, select box, button, slider, and star question types.

⚠️ Important: The notifications aren’t sent to your CHR inbox, but to your email address or mobile number added in your CHR profile. See Updating your user profile information. Only the provider associated with the Qnaire receives the alerts.

Providers can also receive alerts based on a patient's Qnaire score increasing or decreasing compared to previous responses. See Configuring provider alerts based on patients’ Qnaire score changes.

Steps

1. From the main menu, click Qnaires and select a Qnaire.

2. In the Qnaire builder, click the Questions tab, and select the question you want to set up an alert for.

3. Select the response option you want to configure an alert for.

4. Select Alert message to designated provider.

💡 Tip: You can add text in the box below. This text appears in the email or SMS alert.

📌 Note: The notification includes the patient name, gender, age, and the question and response with or without the customized message.

4. Click Close Choice.
