You can record Qnaire responses directly from an encounter. This is helpful when patients have not completed pre-visit Qnaires. There are also some Qnaires that require a provider to complete the Qnaire on behalf of or with their patient.

Administrators can also configure Qnaires to help you write clinical notes, since Qnaires can update patient data and natural language can be used to pre-populate your encounter note with patient reported information.

Steps

  1. Start or open an encounter. Refer to Starting an encounter.

  2. In the top-right corner of the Qnaires section, click + Collect Data. The Add Qnaires window appears.

    ๐Ÿ“Œ Note: If your Qnaires are grouped into folders, the Select Folder window appears. For more information, refer to Organizing Qnaires into folders.

  3. From the Add Qnaires window, select the required Qnaires.

    ๐Ÿ’ก Tips:

    • Numbers next to your selected Qnaires indicate how many Qnaires you've selected and the order in which they open.

    • If you have a long list of Qnaires, you can search the list by keywords. In the Search Qnaires field, start typing the Qnaire name and the list automatically shows the matching Qnaires, including any you've already selected.

  4. Click Start Qnaires and the first Qnaire opens.

    ๐Ÿ’ก Tip: If you have French enabled in your CHR and you have bilingual Qnaires, the Qnaire opens in the last language you viewed a Qnaire in. You can change the language before completing the Qnaire.

  5. Click Start Questionnaire and complete the selected Qnaires. Once completed, the Qnaires are automatically saved to the Qnaires section of the patient's chart and attached to the Qnaires section of the encounter.

    ๐Ÿ“Œ Note: If your Qnaires contain natural language, the Auto-Populate Encounter window appears.

    • If you don't want to add the content to your encounter note, in the Attach Paragraph to list, choose None.

    • To add the content, in the Attach Paragraph to list, select History, Examination or Assessment and Plan.

    • Click to add the content to the Bottom or Top of existing data in that section.

    • Click Attach.

    ๐Ÿ’ก Tip: If you always attach natural language in the same way, you can configure a default section and placement. For more information, refer to Customizing your encounter settings.

  6. To remove a Qnaire response from the encounter, click the remove icon. The Qnaire remains in the Qnaire section of the patient chart.

  7. Complete the rest of your encounter as per usual. Refer to Recording encounter notes.
