Adding response options to your Qnaire questions

When you add Multi-select, Radio, Select Box, Button, and Slider questions to your Qnaire, you must add response options to these questions.

You can create follow-up questions in response to a patient's answer. For example, you can ask a patient a Yes or No question and if they answer Yes, a follow-up question is asked, related to the answer. See Creating follow-up questions to patient responses.

📌 Note: When you add a question to a Qnaire, by default, four empty New Option fields appear under the main question. To delete the ones you don’t use, hover over the blank New Option box, click X on the right, and Okay to confirm the action. To add more options, click +New Choice.

Steps

1. From the Questions tab, add a new question or scroll to an existing question, click New Option. The field expands.

Complete the fields as follows:

Enter Choice

Type the response option the patient can select when answering the question.

📌 Note:

- For button type questions, the option appears on the button.

- For slider type questions, you can choose to show or hide the response options.

Score Modifier

Type a positive or negative value. See Adding score modifiers to questions in Qnaires.

This value is used in the custom score formula you configure for your Qnaire.

📌 Note: You can only configure scoring for Multi-select, Slider, and Radio type questions.

Message Pop-Up on Choice Selection

Type a message that appears when the patient selects this answer.

Natural Language Conversion

You can convert a patient's response into a sentence that appears in an encounter note under the History, Examination, or Plan and Assessment section.

⚠️Important: For the response to appear in the patient’s encounter, the main question must have #answer in the Natural Language Conversion field.

Variable Value

Used for exporting data (simple exports / data slices).

Type a name or code you want to see when exporting data.

Exportable Value

This is used for exporting purposes (simple exports / data slices).

Type the name of the variable as it would appear in the actual cell.

⚠️ Important: This section only appears when you select the button type question.

For button type questions, to add an image to your button:

  1. Click Add pic.

  2. From the file explorer window, select an image and click Open to upload.

Visible HTML (button type only)

📌 Note: This is only applicable for button type question.

To create a unique button:

1. Click Update HTML.

2. In the Update HTML Dialog window, complete the following:

a. Select Enable.
b. Click the Font Size field to change it.

c. Type a colour code in the Font Color field. Default code is black.

d. Click the Text Align field to change the alignments of the text.

e. Click the Width or Height fields to change the size of the button.

f. Type a colour code in the Background Color and Border Color field.

💡 Tip: The primary colour codes are:

  • Red: FF0000

  • Blue: 0000FF

  • Green: 00FF00

  • Yellow: FFFF00

g. Type the HTML code in the Type your text field.

3. Click Save.

Select all

Select the field if you want the response option to select all options of the question.

For example, name the response option All of the above.

Unselect all

Select the field if you want the response option to select none of the options of the question.

For example, name the response option None of the above.

Enable Notice

Override Natural Language

Select if you don't want the patient's response to appear in the encounter notes.

For example: You have the response "All of the above" as an answer to pull into the natural language sentence.

If you want other responses to pull into the natural language sentence in the encounter, the Natural Language Conversion field must be filled out with all the other options. For example: "headache, stomach ache, fever”.

Doing so, all options pull into the encounter note except the "All of the above" option.

Trigger new questionnaire(s)

Select if you want the patient to complete a new Qnaire when they choose the specific response option. The patient is prompted to complete a second Qnaire after completing the first.

Click Qnaire IDs and enter the unique ID of the Qnaire that opens once triggered.

Alert message to designated provider

Select if you want to notify the selected provider that the patient chose this option.

Type a message in the text box below. This is optional.

📌 Note: This message is sent via an email and SMS to the provider's email address and mobile number associated to their profile.

Update Patient Data

Select this if you want patient responses to update their chart. For example, date of birth, gender, etc. See Linking Qnaire responses to patient data for more information.
