When you add Multi-select, Radio, Select Box, Button, and Slider questions to your Qnaire, you must add response options (possible answers) to these questions.

You can create follow-up questions in response to a patient's answer. For example, you can ask a patient a Yes/No question and if they answer Yes, another question is asked, related to the answer. See Creating follow-up questions to patient responses.

πŸ“Œ Note: Four empty New Option fields appear by default. If you do not need all four, delete the empty ones by selecting one and clicking X on the right of the filed box, and Okay to confirm. To add more choice options click +New Choice.

Steps

1. In the selected question, click New Option or + New Choice. The field expands.

This values is used in the custom score formula you configure for your Qnaire.

Message Pop-Up on Choice Selection

Type a message that appears when the patient selects this answer.

Natural Language Conversion

See Setting up natural language. This allows you to convert a patient's response into a sentence that appears in an encounter note under the History section.

Variable Value

This is used for exporting purposes (simple exports / data slices).

Type the name of the variable as it would appear in a data export.

Exportable Value

This is used for exporting purposes (simple exports / data slices).

Type the name of the variable as it would appear in the actual cell.

For button type questions, to add an image to your button,

  1. Click Add pic.

  2. From the file explorer window, select an image and click open to upload.

To create a unique button,

1. Click Update HTML.

2. In the Update HRML Dialog window, complete the following:

a. Select Enable.
b. Click the Font Size field to change it.

c. Type a colour code in the Font Color field. Default code is black.

d. Click the Text Allign field to change the alignments of the text.

e. Click the Width or Height fields to change the size of the button.

f. Type a colour code in the Background Color and Border Color field.

  • Red: FF0000

  • Blue: 0000FF

  • Green: 00FF00

  • Yellow: FFFF00

g. Type the HTML code in the Type your text field.

3. Click Save.

Enable Notice

Override Natural Language

Trigger new questionnaire(s)

Select if you want the patient to complete a new Qnaire when they choose the choice option. The patient is prompted to complete it at the end of the original Qnaire.

Click Qnaire IDs and enter the Qnaire ID of the Qnaire that opens once triggered.

πŸ“Œ Note: Every Qnaire has a unique ID. To find the Qnaire ID, open the Qnaire and refer to the URL. The URL contains a unique number.

Alert message to designated provider

Select if you want to notify the selected provider that the patient chose this option.

Type a message in the text box below. This is optional.

πŸ“Œ Note: This message is sent via an email and SMS to the provider's email address and mobile number associated to their profile.

Update Patient Data

Select, if you want to update patient data in the patient chart. See Linking Qnaire responses to patient data for more information.
