There are several ways to send Qnaires to your patients from the CHR. The simplest way is manually sending it from the patient chart; however, you can also automate the process to cut down on admin work and make sending Qnaires to your patients more efficient.
When you send a bilingual Qnaire, the Qnaire opens in the same language as the Qnaire notification (the patient’s notification language). If the notification language is not set, the system follows a specific sequence to determine the display language. It first checks your preferred language, then your other spoken languages, and finally refers to your clinic's account language. If none of these are set to English or French, or if no language is specified, the system defaults to sending notifications in English.
The patient can also change the language before completing the Qnaire.
📌 Note: If a patient switches languages after responding to some questions, they are warned they will lose their responses.
The table below shows the different ways you can send Qnaires to your patients.
Method | Description |
Linked to appointments | You can associate Qnaires with an appointment type or presenting issue. The Qnaire is then sent with appointment notifications or reminders. This allows you to automate delivery of pre-visit or post-visit Qnaires. |
Attached to appointment type reminders | You can attach Qnaires to a specific appointment reminder in the Appointment type. This allows you to send the Qnaire in only one of the appointment notifications. For example, an appointment with a 1 day prior and 1 hour prior reminder can have a COVID screening Qnaire attached only to the 1 hour prior reminder. |
Attached to a booked appointment | You can manually attach a Qnaire to a booked appointment. You can attach a Qnaire in the Pre-Visit Qnaires tab to send with all notifications, or in the Reminders tab to send with a specific reminder. |
Scheduled | You can configure a Qnaire to automatically send at a date and time that you choose.
Scheduled Qnaires can also be linked to an encounter note or encounter template and then sent at a specific time after the encounter is signed. This allows you to easily send a follow-up Qnaire.

Recurring
| You can send Qnaires to your patients on a recurring basis at a pre-defined interval.
For example, automatically send an Anxiety Self-Assessment Qnaire every week to track progress.

A different Qnaire | You can configure a Qnaire to send based on the patient's response in a different Qnaire. The patient is prompted to complete the new Qnaire after completing the original Qnaire.
